Luther Vandross, Hypertension, Type 2 Diabetes & Stroke

Yesterday I was interviewed about my experience finding my boss, Luther Vandross after he suffered a stroke in his apartment in 2003.

My memories of that day are still crystal clear. My experience fuels my passion for helping others at risk, affected by and living with diabetes, to ‘keep their house a home,’ specifically by learning how to prevent a diabetes health-related complication.

My learning curve about stroke prevention was dramatic, and I don’t wish that to be the case for anyone else.

Two minutes after Luther, who was lying on a gurney, was wheeled into emergency, the doctor came out to tell me that Luther’s stroke could be prevented. I was dumbfounded. Up until that moment, I didn’t know the link between stroke and mismanaged diabetes. I use ‘mismanaged diabetes’ because well-managed diabetes is the leading cause of nothing. In Luther’s case, he was not managing his type 2 diabetes when he suffered a stroke. Instead, numerous people and media like to solely blame his weight for the stroke. They never mention noncompliance as a risk factor too.

I assume Luther must have felt the sense of overwhelming failure after regaining 125 + pounds. Understandably, he might have avoided taking his diabetes medications, seeking out his healthcare providers, and asking for help from others. As a result, he was alone in his battle. I think that was a mistake.

Luther Vandross, at age 52, suffered a stroke on April 16, 2003. But, unfortunately, his healthy story is not unique. He suffered from diabetes and hypertension, and his weight seesawed between 180 and 320 pounds. Even the term ‘yo yo’ dieting seems a bit naive to me. Personally, I feel Luther suffered from disordered eating behavior. Most of us don’t take time to think about our eating habits like what, when, and why they eat

Signs and symptoms of disordered eating may include, but are not limited to:

  • Frequent dieting, anxiety associated with specific foods, or meal skipping
  • Chronic weight fluctuations
  • Rigid rituals and routines surrounding food and exercise
  • Feelings of guilt and shame associated with eating
  • Preoccupation with food, weight, and body image that negatively impacts the quality of life
  • A feeling of loss of control around food, including compulsive eating habits
  • Using exercise, food restriction, fasting, or purging to “make up for bad foods” consumed

Disordered eating behaviors are treatable with the right resources and help. But many people who suffer from disordered eating patterns either minimize or do not fully realize the impact it has on their mental and physical health. Luther liked to joke about his eating behaviors and weight. Specifically, I feel Luther suffered from binge eating. 

Binge eating disorder (BED) is characterized by recurrent episodes of eating large quantities of food (often very quickly and to the point of discomfort); a feeling of a loss of control during the binge; experiencing shame, distress or guilt afterward; and not regularly using unhealthy compensatory measures (e.g., purging) to counter the binge eating. It is the most common eating disorder in the United States.

Sadly, Luther and I never discussed his type 2 diabetes beyond the need to refill a prescription. I didn’t even know he had hypertension. So do I regret that I wasn’t more involved in his diabetes self-care? Absolutely! But it seems only Cher is capable of turning back time. So my time left on earth is dedicated to sharing my experience to inspire others. 

We did discuss his weight. He asked me how I managed my weight. I told him that I exercised. When he said that he didn’t exercise, I suggested that he try step aerobics. Up until that point, Luther relied on liquid diets to maintain his weight loss. Ultimately, Luther liked step aerobics so much that we traveled on tour with a step aerobics instructor.

Unfortunately, his all-or-nothing approach to diet and exercise did not help. He could not practice moderation. I don’t think he is alone in that regard. My friend, High Voltage, abstains from added sugar because she believes her body reacts to added sugar like it’s a drug. She often compares the experience of sugar cravings to cocaine addiction. High Voltage can’t have one teaspoon of sugar and stop. Unfortunately, moderation is not the cure for everyone. Sadly, we choose to beat ourselves up and others for lacking the willpower to stop eating certain foods when the real culprit could be brain chemistry.  

During the interview, I was asked if Luther experienced love. Unfortunately, I think he was lacking in self-love.  

“If I’m emotionally distraught, then eating is my coping mechanism,” Luther Vandross told Britain’s Q magazine in 1991. “For what ails me, it seems to be the only thing that takes the edge off the pain.”

Strokes, the third-leading cause of death for all racial and ethnic groups, killed 18,884 African-Americans in 1999.

Also, in 1999, 11,927 African-Americans died from diabetes, the sixth-leading cause of death for African-Americans.

By the time Luther Vandross headed to the studio to record his fifth album Give Me the Reason in 1986, he’d become one of the most successful soul singers of the first half of the eighties. His four previous albums have been either certified platinum or double-platinum in America.

Practice What You Preach: High Blood Pressure

Whether your doctor warned you about your high blood pressure at your last visit or you tested it yourself at a pharmacy, it should be taken seriously. According to the Mayo Clinic, “The higher your blood pressure and the longer it goes uncontrolled, the greater the damage.”

Barry White battled chronic high blood pressure, which caused kidney failure.

When the blood vessels become damaged, the nephrons that filter your blood don’t receive the oxygen and nutrients they need to function well reports the American Heart Association. This is why high blood pressure (HBP or hypertension) is the second leading cause of kidney failure. Over time, mismanaged high blood pressure can cause arteries around the kidneys to narrow, weaken or harden. These damaged arteries are not able to deliver enough blood to the kidney tissue.

Damaged kidney arteries do not filter blood well

When the arteries become damaged, the nephrons do not receive the essential oxygen and nutrients — and the kidneys lose their ability to filter blood and regulate the fluid, hormones, acids, and salts in the body.

Damaged kidneys fail to regulate blood pressure.

Healthy kidneys produce a hormone called aldosterone to help the body regulate blood pressure. Kidney damage and mismanaged high blood pressure each contribute to a negative spiral. As more arteries become blocked and stop functioning, the kidneys eventually fail.

“Doctors confirmed that Barry White had a history of hypertension [high blood pressure], which led to a decline of kidney function,” a statement from Island Def Jam Music Group said.

In a career spanning 30 years, the 58-year-old Barry White has sold 100 million albums. He has twice been admitted to hospital in recent years suffering from hypertension – once in 1995 and again in 1998.

How To Correctly Monitor Your Blood Pressure At Home

“When you have high blood pressure your heart is working overtime, all the time, “says Neva White DNP, CRNP, CDE on October’s Diabetes Late NIte podcast. “Measuring your blood pressure is one of the best  non invasive tests that we have available to give us a window into how well the heart is actually functioning.”

The American Heart Association recommends home monitoring for all people with high blood pressure to help the healthcare provider determine whether treatments are working. Home monitoring (self-measured blood pressure) is not a substitute for regular visits to your physician.

How to use a home blood pressure monitor

Be still. Don’t smoke, drink caffeinated beverages or exercise within 30 minutes before measuring your blood pressure. Empty your bladder and ensure at least 5 minutes of quiet rest before measurements.

Sit correctly. Sit with your back straight and supported (on a dining chair, rather than a sofa). Your feet should be flat on the floor and your legs should not be crossed. Your arm should be supported on a flat surface (such as a table) with the upper arm at heart level. Make sure the bottom of the cuff is placed directly above the bend of the elbow. Check your monitor’s instructions for an illustration or have your healthcare provider show you how.

Measure at the same time every day. It’s important to take the readings at the same time each day, such as morning and evening. It is best to take the readings daily however ideally beginning 2 weeks after a change in treatment and during the week before your next appointment.

Take multiple readings and record the results. Each time you measure, take two or three readings one minute apart and record the results using a printable (PDF) or online tracker. If your monitor has built-in memory to store your readings, take it with you to your appointments. Some monitors may also allow you to upload your readings to a secure website after you register your profile.

Don’t take the measurement over clothes.

Why keep a blood pressure journal?

One blood pressure measurement only tells what your blood pressure is at that moment. A diary of multiple readings taken over time provides a better picture of your blood pressure that can help you partner with your physician to ensure that your treatments  to lower high blood pressure  (HBP or hypertension) are working.

If you have been prescribed medication to lower your blood pressure, don’t stop taking your medication without consulting your doctor, even if your blood pressure readings are in the normal range during home monitoring.

High blood pressure (hypertension) is a leading cause of heart disease and stroke, which together kills more New Yorkers than any other disease.

About one in four New Yorkers has high blood pressure, but there are usually no symptoms. Black and Latino adults are more likely to have high blood pressure than other races/ethnicities.

Blood pressure is the force of blood moving through your blood vessels. The higher it is, the more damage it can do to the inside of your blood vessels. High blood pressure can also lead to problems with your kidneys, eyes and to sexual dysfunction.

Everyone should know their blood pressure. You can get it checked by asking your health care provider, visiting a pharmacy or checking it yourself using a home monitor.

Understanding Your Blood Pressure Numbers

Blood pressure is reported with two numbers, one written over the other. The top number, systolic blood pressure, reflects the amount of pressure in your arteries when your heart squeezes to pump blood. The bottom number, diastolic blood pressure, reflects the amount of pressure in your arteries when your heart relaxes between heartbeats.

For People Not Diagnosed with Hypertension

Systolic (upper) Diastolic (lower) Category and Recommended Action
Below 120 and Below 80 Normal. No action necessary.
120–139 or 80–89 Elevated. Talk with your your health care provider at your normally scheduled visit.
140–179 or 90–109 High. Talk with your health care provider soon.
180 or Higher or 110 or Higher Dangerously High. You could be in danger. Get medical care immediately.

Diabetes damages arteries and makes them targets for hardening, called atherosclerosis. That can cause high blood pressure, which if not treated, can lead to trouble including blood vessel damage, heart attack, and kidney failure.

There are several ways in which high blood sugar levels in the blood can increase blood pressure:

  • The blood vessels lose their ability to stretch.
  • The fluid in the body increases, especially if diabetes is already affecting the kidneys.

The combination of high blood pressure and type 2 diabetes can greatly increase your risk of having a heart attack or stroke.

If your blood sugar level is too high, you may experience: increased thirst, frequent urination an fatigue. However, the American Heart Association (AHA) stresses that most of the time there are no symptoms for high blood pressure (also known as hypertension).

People usually find out they have high blood pressure when a doctor takes a blood pressure reading, or they take one themselves at home.

The reading will give two numbers:

  • The systolic is the top number
  • The diastolic is the bottom number

According to the AHA, the results will be one of the following:

  • Normal: Systolic below 120 and diastolic below 80
  • Elevated: Systolic 120–129 and diastolic under 80
  • Hypertension stage 1: Systolic 130–139 and diastolic 80–89
  • Hypertension stage 2: Systolic 140-plus and diastolic 90 or more
  • Hypertensive crisis: Systolic higher than 180 and diastolic above 120.

What is considered normal blood pressure for people with diabetes?

On the basis of recent studies, most people with diabetes should have a blood pressure that’s more than 130/80 mm.

People with diabetes can help decrease the risk of high blood pressure and cardiovascular disease by managing their blood sugar levels.

  1. Stop smoking
  2. Increase activity and exercise more
  3. Lose weight if you’re overweight
  4. Cut back on sugar, refined carbohydrates and processed foods

Remembering Gerald Levert

Divabetic remembers songwriter, arranger, producer, and performer, Gerald Levert on what would have been his 54th birthday, (7/13/20).

Gerald Levert’s career began  in 1984, when he teamed up with his brother Sean Levert and friend Marc Gordon, to form the band LeVert. A couple of solid albums followed and received attention on the R&B charts – but in 1987, Their big hit, Casanova, took them to the top of the charts in 1987.

In in 1991, Gerald Levert began his solo career with a  string of successes including the operatic “Taking Everything.”

Even when Gerald Levert died on November 10, 2006, his career wasn’t over. His album In My Songs was released posthumously in 2007, and the title track was a Grammy winner – Gerald Levert’s first.

Gerald Levert’s early death – the singer was just 40 years old – was unexpected and devastating to his family, friends and fans.

Gerald Levert’s death was caused by a fatal combination of prescription and over the counter medications according to the coroner’s report.

Andy Gibson, a family spokesperson, said Gerald Leveret was taking pain medications because of chronic pain from a lingering shoulder injury and surgery in 2005 to repair a severed Achilles tendon.

 

From Gerald Levert’s shoulder injury to sports injuries, chronic pain is surprisingly common. About 116 million Americans are estimated to struggle with chronic pain according to The Ranch Tennessee. While there are many ways to manage it, some individuals turn to prescription painkillers. Opioids, such as Vicodin, OxyContin and Percocet, to relieve the discomfort.

Many patients are able to take prescription painkillers without becoming addicted. However, some people don’t follow the prescribed dosage and take larger doses or take the medication more frequently.

Warning Signs of Addiction

  • Requiring increasingly larger doses to achieve the same effect
  • Seeking prescriptions from multiple physicians (also called “doctor shopping”)
  • Changes in personality, including mood or energy levels
  • Withdrawing from family and friends
  • Neglecting responsibilities, including work and household chores
  • Becoming defensive when a loved one brings up the potential problem
  • Deteriorating appearance and declining personal hygiene habits

The smooth R&B programming nicknamed ‘the Quiet Storm’ starting in 1976 and came to prominence in the mid-80s, breaking artists including Luther Vandross, Anita Baker, and Sade, and establishing hit-makers like Jimmy Jam and Terry Lewis and LA Reid and Babyface. It was an alternative to funk, disco, and boogie that also gifted “old-school” R&B artists with the extended careers that classic rock artists enjoyed.
An urban alternative to soft rock or easy listening, Quiet Storm ignores most of the programming rules of commercial radio. Songs can be current or decades old, deep cuts or singles, and are more likely to be a live version or extended length than a radio edit. Instrumentals also get burn; jazz fusion is a favorite.

“My aim is to empower quiet storm music fans to learn more about the health conditions affecting their favorite artists and apply it to their own lives,” says Max Szadek. “For example, Quiet Storm legend, Barry White suffered from chronic hypertension, like many of his fans,  which caused severe kidney damage before his death. In fact, the prevalence of high blood pressure (HBP or hypertension) in African-Americans in the United States is among the highest in the world. More than 40 percent of non-Hispanic African-American men and women have high blood pressure. For African-Americans, high blood pressure also develops earlier in life and is usually more severe according to the American Heart Association.
